Day 1 — Surgery. All blood tests and the medical examination are carried out at the clinic on the day of the procedure — no advance preparation needed. This is critical: lying flat causes the anaesthetic fluid to migrate downward from the forehead into the periorbital area, causing swelling around the eyes. Swelling on Days 2–4 is completely normal — the 45° position significantly reduces it. Do not sleep on your side or stomach for the first 5 days. Avoid bending forward, walking for long periods, or standing — these increase facial swelling. No alcohol, cigarettes, or hookah for at least 7 days. No coffee or caffeine for the first 3 days. Avoid salty foods (olives, pickles, crisps) and added salt in cooking for the first 5 days.

Day 3 — Clinic Visit. You return to Estethica for scheduled procedures: infrared therapy and laser treatment reduce swelling, accelerate tissue healing, and improve blood circulation in the scalp. A clinic nurse then performs the first professional hair wash and teaches you the correct technique you'll use at home. Then — transfer to the airport.

Your Medication: What Estethica Prescribes and Why

Duocid 375 mg (amoxicillin + clavulanic acid) — broad-spectrum antibiotic. Take twice daily with meals for 5 days. Protects the transplant and donor zones from bacterial infection while the skin barrier is compromised. Do not skip doses — an incomplete antibiotic course reduces effectiveness.

Aprol Fort 550 mg (naproxen) — anti-inflammatory and painkiller. Take only when needed, no more than 3 times per day. Bring with you to your Day 11 clinic visit.

Prednol 16 mg (methylprednisolone) — corticosteroid. Take once daily. This is the key medication against periorbital swelling: corticosteroids suppress the inflammatory response and significantly reduce swelling around the forehead and eyes on Days 2–4. Bring with you to your Day 11 clinic visit.

Bepanthol lotion — dexpanthenol, accelerates tissue regeneration and soothes skin irritation. Apply once daily to the donor area. Bring with you to your Day 11 clinic visit.

Sebamed shampoo — pH-balanced shampoo with an acidity level of 5.5, matching the natural pH of your skin. Does not disrupt the scalp's protective barrier and is suitable for sensitive skin during the healing period.

Periorbital Swelling After a Hair Transplant: Why It Happens and When It Resolves

One of the most common concerns patients raise on Days 2–4: "Why are my eyes swollen? Is this normal?" Our medical expert Dr. Oleg Filatov explains.

"Periorbital oedema — swelling of the soft tissue around the eyes — following a frontal zone hair transplant is a completely predictable and physiologically explainable phenomenon. Here's the mechanism: during surgery, a local anaesthetic solution combined with saline is injected under the scalp to facilitate hydrodissection of the tissue and ease graft implantation. After the procedure, this fluid gradually migrates downward under gravity — from the forehead into the periorbital area, producing the characteristic swelling around the eyes. At the same time, the body's inflammatory response is at work: any surgical intervention triggers the immune system — capillaries dilate, blood flow to the operative zone increases, and fluid accumulates in the surrounding tissue. The important thing to understand is this: periorbital swelling is not a complication or an allergic reaction — it is a normal physiological process. It typically peaks on Days 3–4 and resolves completely on its own by Days 7–10. Factors that worsen it: sleeping horizontally, excess salt in the diet in the first few days, physical activity, and bending forward. This is precisely why sleeping at 45°, restricting salt, and maintaining rest for the first 5 days are not just recommendations — they are direct measures against swelling. Methylprednisolone (Prednol), taken as prescribed, additionally suppresses the inflammatory component and significantly reduces both the severity and duration of periorbital oedema. If swelling increases after Day 5 or is accompanied by pain or redness, contact the clinic immediately."

How to Wash Your Hair After a Hair Transplant: Estethica's Day-by-Day Protocol

Why does washing technique in the first 10 days directly affect graft survival?

Days 3–10 — Daily washing:

Pour shampoo into the palm of your hand and work it into a foam. Apply the foam to the transplanted area using gentle patting movements — no pressure, no rubbing, no circular motions. Apply lotion to the donor area (the back of the head) with massaging movements. A bath sponge works well — squeeze it out and use it to distribute foam evenly across the scalp without direct hand contact. Rinse with a gentle, warm stream of water — never under direct shower pressure. Do not rub with a towel afterwards — only pat dry gently. Do not remove the headband while washing — you can let it rest around your neck.

Day 11 — Final wash (at the clinic or at home):

This wash is different. First, apply lotion to the transplanted area and leave it on for 60 minutes. Rinse with warm water. Then, using a gauze pad or bath sponge, apply shampoo with gentle patting movements from the back of the head toward the hairline. After Day 10, you can gently scrub off any remaining crusting — the grafts are now permanently fixed in the skin and cannot be dislodged. By Day 13, all scabs must be completely removed: this is critical for the follicles to "breathe" and function properly.

During washing you will notice hair follicles coming away from the transplanted area — this is a normal process. The grafts are anchored in the skin and cannot be removed.

For the first 5 days, sleep exclusively on your back at a 45° angle, using a neck pillow or rolled towel for support. Your scalp must not press against the pillow surface. Sleeping on your side or stomach in the early days puts direct pressure on the grafts at the most vulnerable stage of fixation.

From Day 6, you can gradually return to your normal sleeping position — but treat your scalp gently for at least another 2 weeks.

— Lifting heavy objects — avoid for 7 days

— Activities that cause sweating — avoid for 7 days

— Light sport — avoid for 10 days

Why does sport matter? Physical exertion raises blood pressure and increases sweating — creating a direct risk of infection and graft displacement during the healing phase.

Water:

— Hot shower, hammam, sauna — avoid for 15 days (warm water only)

— Sea, swimming pool — avoid for 2 months

Chlorine, seawater and overheating irritate the healing scalp, soften scabs prematurely and increase infection risk.

Hairstyling:

— Scissors on the sides — from Day 10

— Hair dryer, hair dye — after 3 months

— Gel, wax, pomade, spray — after 3 months

— Shaving machine or razor — after 3 months

— Wigs or hairpieces — never

Intimacy — avoid for the first 3 days.
